The header block is written for teams, and golf is not one, so read it loosely: Team A and Team B are your two columns, which means two players in a singles round, or two sides in a fourball. Date and Venue take the course name — Venue is worth filling properly, since the same card means different things at two courses. Referee and Scorekeeper are where the marker goes: in club stroke play the person who keeps your card is a fellow player, and the signature block at the foot is where the two of you countersign it.

Below it, eighteen rows: Hole 1 to Hole 18, one box per player. Write the strokes actually played on the hole — the raw number, not the score relative to par. There is no par column printed, so if you want the reference under your eye, pencil each hole's par next to its number before you tee off; par 3, 4 or 5 depending on length. The Final score row adds the column up, and this is the one grid on the site where the small number wins: fewest strokes takes the round.

Penalty strokes belong inside the hole's box, never in a separate column, because a penalty stroke is a stroke. Out of bounds or a lost ball costs stroke and distance: add one and replay from where you played before. A ball in a penalty area is relief at one stroke, and grounding your club in the sand before playing costs two in stroke play. Add them as you go, hole by hole: a card rebuilt from memory at the nineteenth is a card that no longer matches your marker's, and two cards that disagree cannot both be signed.

The values strip is unusually plain here: +1 stroke and +2 strokes, because in golf you are not awarding points, you are counting work done. The sanctions strip carries a single word, Penalty, and in the app that counter adds a stroke to the total exactly like any other — which is the honest way to model it. Name each hole against its par as you write: one under is a birdie, one over a bogey, two under an eagle. The names are for the story; the box keeps the raw strokes.

Two things to keep an eye on that the sheet cannot check for you. The bag: fourteen clubs is the ceiling for a round, and the penalty for a fifteenth is capped but real. And the format: everything above assumes stroke play. In match play you do not add strokes at all — you win or lose each hole, and the match stops the moment it can no longer be levelled, so the Final score row holds holes won, not strokes. Decide which one you are playing on the first tee, not the tenth.
